It is long, so people that fill it out must have a really good incentive to complete it. I also found questions on page 6 confusing. It was hard for me to grasp that 0..5 are different scales. On one side 5 is "very easy" and the next questions it is "very important". Probably, add some separators between pair of questions, so once people figure out the first question they can use that knowledge to answer the rest.
Overall, I would shorten it to answer your one "most scariest question". With this survey, you'll have plenty of data, but I doubt there will be many insights from it. Just think about how you're going to analyze results.
Thanks for the feedback! I remove those 1 to 5 prefixes, because it was unnecessary. Unfortunately Google forms does not support (or at least I could not figure out) these kind of separation. Trying to figure out how to make it shorter.
